This can be alarming, but in most cases it is a temporary and well-recognized phenomenon associated with significant weight loss rather than a direct side effect of the medication itself.

Understanding the Hair Growth Cycle

Hair grows in a cycle with three main phases:

  • Anagen (growth phase): The active growing phase, lasting two to seven years for most scalp hair.
  • Catagen (transition phase): A brief transitional period lasting about two weeks.
  • Telogen (resting phase): The hair rests for about three months before shedding and being replaced by new growth.

At any time, roughly 85–90% of scalp hair is in the anagen phase, and 10–15% is in the telogen phase. Normal daily shedding is approximately 50 to 100 hairs.

What Is Telogen Effluvium?

Telogen effluvium is a condition in which a larger-than-normal proportion of hair follicles shift into the telogen (resting) phase at the same time, leading to increased shedding approximately two to three months after a triggering event.

Telogen effluvium is not the same as male-pattern or female-pattern hair loss (androgenetic alopecia), which involves gradual thinning over years. Telogen effluvium is typically diffuse — meaning it affects the whole scalp rather than creating bald patches — and is usually temporary.

Why GLP-1 Treatment May Trigger Shedding

GLP-1 medications themselves are not generally considered a direct cause of hair loss. However, the changes that often accompany treatment can trigger telogen effluvium:

  • Rapid weight loss: Significant weight loss over a short period is one of the most common triggers for telogen effluvium. The body may redirect nutrients away from non-essential functions like hair growth during periods of caloric deficit.
  • Reduced caloric and nutrient intake: Eating substantially less can reduce intake of protein, iron, zinc, biotin, and other nutrients that support hair health.
  • Stress on the body: Any significant physical stress — including rapid weight loss, illness, surgery, or hormonal shifts — can prompt follicles to enter the resting phase.
  • Other simultaneous changes: Starting a new medication, dietary change, or exercise routine at the same time can contribute.

The shedding typically begins two to three months after the triggering event, which is why people often connect it to GLP-1 treatment even though the medication may not be the direct cause.

How Long It Lasts

In most cases, telogen effluvium resolves on its own within six to nine months after the triggering event. Hair typically regrows once the underlying stressor — such as rapid weight loss — has stabilized and nutrient intake is adequate.

If shedding continues beyond several months, is accompanied by bald patches, scalp irritation, or other symptoms, it may indicate a different type of hair loss that warrants evaluation.

What You Can Do

If you are experiencing hair shedding during GLP-1 treatment:

  • Talk to your healthcare team. They can help determine whether the shedding is likely telogen effluvium or whether further evaluation is needed.
  • Maintain adequate nutrition. Ensure you are getting enough protein, iron, zinc, and other essential nutrients. A clinician or registered dietitian can help assess whether your intake is adequate.
  • Avoid rapid, extreme weight loss. Gradual weight loss is less likely to trigger shedding than very rapid loss.
  • Be patient. Hair regrowth takes time. Shedding that started months ago may continue for a while even after the cause is addressed.
  • Consider blood work. Your clinician may recommend checking for common nutritional deficiencies, such as iron, vitamin D, or thyroid function, which can contribute to hair shedding.

When to Seek Further Evaluation

Consult a healthcare professional or dermatologist if you experience:

  • Shedding that continues beyond six to nine months
  • Bald patches or visible scalp thinning in specific areas
  • Scalp redness, itching, scaling, or pain
  • Hair loss accompanied by other symptoms such as fatigue, weight changes, or skin changes

These may indicate conditions other than telogen effluvium, such as androgenetic alopecia, alopecia areata, or an underlying medical condition.
