Checking GLP-1 Coverage Through Your Employer Health Plan

Employer health-plan coverage can differ even when two people have insurance administered by the same company. Your employer’s specific plan may cover certain medications, require additional approval, or exclude medications used for weight management.

Start with these steps

  1. 1

    Review your plan information

Start with the documents and portals your employer or insurer provides. Look for the prescription drug formulary, covered-drug list, and benefit summaries.

    • Prescription drug formulary
    • Covered-drug list
    • Summary Plan Description
    • Summary of Benefits and Coverage
    • Prescription-benefit information
    • Employer benefits portal
  2. 2

    Search for the exact medication

Search the exact medication being considered or prescribed. Note what the plan says about it.

    • Covered or not covered
    • Preferred or nonpreferred tier
    • Prior authorization
    • Step therapy
    • Quantity limits
    • Required or preferred pharmacy
  3. 3

    Call the prescription-benefit number

Use the member-services or pharmacy-benefit number on your insurance card or plan portal to confirm what you found online.

  4. 4

Contact the employer benefits office when necessary

Ask whether the employer plan broadly excludes anti-obesity or weight-management medications. The insurance company does not always control the employer’s benefit design alone.

Questions to ask

  1. 1Is this exact medication covered under my specific plan?
  2. 2Is it covered for the reason it was prescribed?
  3. 3Is prior authorization required?
  4. 4Is step therapy required?
  5. 5Is there a quantity limit?
  6. 6Must I use a particular pharmacy?
  7. 7Does my deductible apply?
  8. 8What is my expected out-of-pocket cost?
  9. 9Does approval expire or require renewal?
  10. 10Does the plan exclude weight-management medication?

If coverage is denied

  1. 1Read the written denial and identify the exact reason.
  2. 2Give the denial information to the prescribing office.
  3. 3Ask whether missing information or prior-authorization documentation can be corrected or resubmitted.
  4. 4Request the plan rule or coverage criterion used for the decision.
  5. 5Follow the internal appeal instructions and deadline in the denial notice.
  6. 6Ask whether external review is available after an unsuccessful internal appeal.
  7. 7When a broad employer-plan exclusion applies, ask the employer benefits administrator whether any exception process exists under the plan.

GLP-1 Primer does not promise that an appeal will succeed.
