If coverage is denied

First identify whether the issue is a prescription-drug denial by the health plan, a Marketplace eligibility or enrollment decision, missing information, a nonformulary medication, a plan exclusion, or a prior-authorization requirement. Marketplace eligibility appeals and drug-coverage appeals are separate processes.

  3. 3Ask whether a prior authorization, formulary exception, or corrected submission is appropriate.
  4. 4Follow the plan’s internal appeal process.
  5. 5Review whether external review is available.
  6. 6Follow the deadlines in the written notice.

For a Marketplace eligibility or enrollment decision, follow the official Marketplace appeal instructions rather than the prescription-drug appeal process. Do not combine these two appeal systems.
